I read some document (can't remember what/where) that if you take ivm with a fatty meal, you get 2.5x the absorbtion into your blood stream compared to taking it on an empty stomach. If you take it on an empty stomach, there is more left behind to do its work on your gut parasites. So WHY you take it dictates HOW you should take it.

"the high lipid solubility of ivermectin resulting in it being widely distributed throughout the body. Following oral administration, mean peak plasma concentration occurs approximately 4 h after dosing, a second peak at 6–12 h probably arising because of enterohepatic recycling of the drug, with the plasma half-life of ivermectin being around 12 h"
The high lipid solubility certainly seems to suggest taking with a meal
